Football 2025 Requirements:

• Completed Online Registration Form and payment
• AYF Packet Completed and Returned
• Sports Physical (Not NC health assessment for school they are not the same.)
  
• State ID from NC DMV, Military ID or Passport

IMPORTANT NOTE:

State ID, Military ID or passport will remain in the book until the end of the season. It is recommended that you obtain a state ID, that can remain with the league until the END of the season. ID's will be returned after league equipment has been returned. No player can play WITHOUT an ID.  Please obtain this prior to registration. 

Note: DMV requires Social Security Card and Birth Certificate

Cheer 2025 Requirements:
